A whole system of care: How DispatchHealth and our partners work together

Technology has fueled one of the top trends of the 21st century: precision curation of the right services to the individuals who need them. Healthcare providers and their patients have been among the chief beneficiaries of this trend, as new ways to deliver medical services fill the gaps between hospital or nursing home care and traditional doctor’s office consultations.

DispatchHealth is proud to be at the forefront of this trend by bringing many of the medical services provided in emergency rooms and urgent care clinics straight to our patients’ homes. For patients, this eliminates the need to travel and spend time in a waiting room when they’re stressed and not feeling well. It’s also a plus for senior living communities and home health agencies, as they have a reliable partner to call for on-site urgent care services that can often eliminate the hassles of transporting residents to a medical clinic.

There are benefits for hospital and emergency room staff as well. Why? Because when DispatchHealth provides in-home medical services, it reduces the strain on resources that occurs when patients use the emergency room for non-emergent health concerns.

How our in-home health services fit into a whole system of care

DispatchHealth can provide acute medical care on-demand for patients in the comfort of their homes while also working with their primary care physicians and specialists to make sure they’re fully informed every step of the way. They offer same-day, in-home medical treatment for urgent illnesses and injuries that don’t require an emergency room visit. Some of these include:

  • Pneumonia
  • Viral infections such as colds, flu, and COVID-19
  • COPD and asthma exacerbations
  • Congestive heart failure
  • Urinary tract infections (UTIs)
  • Sprains, strains, and minor fractures

Advanced Care and Bridge Care

Sometimes patients recovering from an acute illness or injury require an advanced level of clinical care for multiple days to decrease the risk of readmission. DispatchHealth offers two healthcare models to achieve this goal:

1) Advanced Care, an in-home hospital alternative with 24/7 monitoring. Patient is cared for by a hospitalist-trained internist, supported by a team of advanced practice providers and a nurse care coordinator. Service includes daily rounding, nursing, remote patient monitoring, physical therapy, care coordination, ADL support, and meal prep.

2) Bridge Care, a readmission avoidance service that offers high-risk patients an in-home visit 24 - 72 hours post-discharge. A medical technician and a physician assistant or nurse practitioner, virtually supported by an emergency medicine physician, create a care plan that ensures a smooth, safe transition from hospital to home.

Mobile imaging

DispatchHealth’s mobile medical teams travel with many diagnostic tools at their fingertips, including in-home imaging services. Through a partnership with PPX, Dynamic Mobile Imaging, and other radiology service providers, we can deliver hospital-grade X-ray, ultrasound, Doppler, and bone density imaging.
