We’ll work with you and your main provider on a care plan that meets your needs right at home, whether that’s just one visit to help you settle in, or a little extra support while you recover.

What to Expect from a Recovery Care at Home Visit

We come to your door with care tailored to your experience and needs. In a typical visit, we’ll:

Review your discharge plan with you and your caregivers or family members to make sure everything’s crystal clear.

Double-check your medicine list and make sure you’re taking them correctly. (This gives you extra security against one of the most common sources of medical error.)

Answer any other questions that are on your mind.

Check that your recovery is going well and help manage any ongoing or new symptoms.

What will I pay for Transitional Care at Home?

The cost of a Transitional Care visit is typically the same as an in-network office visit. We’ll bill your insurance, and your insurance plan will determine the percentage of the total bill for which you are responsible. As with any healthcare service, the amount you owe may vary depending on whether you’ve met your deductible, so it’s a good idea to check with your health plan.

Transitional Care always begins with a visit from a healthcare professional to your home. Based on your needs, members of your care team will continue to provide care in person or remotely throughout your Transitional Care support. An emergency medicine doctor is always on call.

Does Transitional Care at Home replace my other medical appointments or ongoing care with my doctor?

No. Transitional Care is designed to provide a period of extra, in-home support immediately following a health episode — a type of care that your usual doctor or specialists may not be able to provide. We coordinate with your usual care team by sharing a recap of your visit, so they’ll be up to speed on all the details of your recovery when you go for your follow-up appointment. Research shows that patients who receive dedicated recovery support at home are far less likely to end up back in the emergency room or hospital.
