Who will come to my home?

We employ the same medical experts you’d see in a hospital, including doctors, nurse practitioners, physician assistants and care coordinators. They give you 100% of their attention from the moment they step through the door––no rush and no distractions.

Who is eligible for Hospital-Alternative Care?

Our medical professionals will help you decide if Hospital-Alternative Care is the right option for your needs. If you’re dealing with any of the following issues, we can help.

  • COPD or CHF exacerbations
  • Nutritional and metabolic disorders

Does Hospital-Alternative Care replace my doctor?

No. Coordination with your primary care doctor is a cornerstone of our program. We work directly with your doctor to determine if Hospital-Alternative Care is right for you. Throughout the program, we continue to communicate and update your doctor.

Does Hospital-Alternative Care replace a hospital?

Yes, for certain conditions that can safely be treated and monitored in the home, Hospital-Alternative Care is another option to an in-patient hospital stay. However, if you are experiencing a life-threatening emergency, call 911 or go to the nearest ER.

How can I become a Hospital-Alternative Care patient?
