Transitional Care begins with a recommendation from your doctor or your health plan. If you feel like Transitional Care might be a good fit for you or a loved one, we encourage you to talk to your healthcare provider.
We’ll work with you and your main provider on a care plan that meets your needs right at home, whether that’s just one visit to help you settle in, or a little extra support while you recover.
What to Expect from a Transitional Care at Home Visit
We come to your door with care tailored to your experience and needs. In a typical visit, we’ll:
Review your discharge plan with you and your caregivers or family members to make sure everything’s crystal clear.
Double-check your medicine list and make sure you’re taking them correctly. (This gives you extra security against one of the most common sources of medical error.)
Answer any other questions that are on your mind.
Check that your recovery is going well and help manage any ongoing or new symptoms.
Make recommendations for next steps and help you address them seamlessly.
