Advanced Care services can be utilized in adult patients for a wide range of medical illnesses that would otherwise result in a hospital admission. The most commonly treated illnesses are: pneumonia and respiratory infections, heart failure exacerbations, cellulitis, kidney and urinary tract infections, chronic obstructive pulmonary disease exacerbations, and electrolyte abnormalities. There are three primary pathways for patients to access Advanced Care:
Pathway 1: A patient who is receiving in-home treatment via a DispatchHealth urgent medical care visit (Acute Care) can be escalated to Advanced Care as an alternative to hospital admission.
Pathway 2: A patient who is being treated by a community provider (primary care or specialist) or through a case manager or nurse triage can be recommended for Advanced Care by contacting our team directly.
Pathway 3: A patient being treated in the ED or hospital can be evaluated for appropriateness to complete their inpatient course of treatment at home through Advanced Care. This care pathway meets all of the requirements to support health system partners who participate in the Acute Hospital Care at Home CMS waiver program.
Dispatchhealth provides Advanced Care in two phases: a high acuity admission stage lasting on average up to 4 days and a transitional care stage of up to 30 days, all of which is built into a single bundle payment. Episode services include in-home provider visits, a physician-led plan of care, mobile point of care advanced laboratory capabilities, mobile imaging, PT/OT, in-home nursing, and 24/7 remote patient monitoring. Advanced Care was designed with patient safety top of mind, with multiple layers of safety assurances built into care delivery. This includes a comprehensive process for the rare instance that a patient does require escalation to a higher level of care.
The DispatchHealth Advanced Care team includes a hospitalist physician, advanced practice provider with inpatient experience, clinical RNs, and RN care navigators. DispatchHealth also offers emergency-trained Acute Care teams, which can provide support care for patients should a concern arise during an Advanced Care episode.
DispatchHealth Advanced Care has the unique ability to directly admit patients into the program from their home, never having to step foot in the hospital to then go home. This is why we have a 98% patient acceptance rate into the program, which is 2.5x higher than traditional health at home programs.
In addition, we can also support a health system’s hospital-to-home program via the Acute Hospital Care at Home CMS waiver program.
DispatchHealth is built on a decade of honing supply chain, logistics, and software implementation that is critical to facilitate reliable, safe, high quality, and timely care. This includes point-of-care moderately complex lab capabilities, mobile imaging, ECGs, and immediate access to supplemental oxygen.
Finally, we can leverage our urgent medical care services, Acute Care, to act as a safety net for unexpected health exacerbations. Our high-acuity trained providers and expansive fleet of vehicles can respond quickly to provide on demand treatment in the home.
Coordination with primary care is a cornerstone of our Advanced Care program. The DispatchHealth Advanced Care team collaborates with primary care providers to provide seamless access to care services, including inclusive huddles when desired, tech-enabled communication, and shared clinical progress notes.
